    a) How did you first hear about the Multimedia Arts and Design Academy?
    What quality or feature did you find most attractive?
    b) Describe one of your most significant experiences and how it has affected your life.
    c) Please describe your college or career plans and how you plan to achieve them.

Required Supplemental Information

Parents of applicants who are not currently enrolled at Santa Barbara High School are required to include a transcript from their child’s current school with this application. 

Please provide three letters of recommendation. At least two recommendations should be from current teachers. Please use the forms provided. Include the person’s name, position, signature, and contact information. These forms can be submitted here.

Please read and Sign the attached “Parents Agreement to Participate” and “Academy Members Learning Agreement.” Which can be found  here. Turn this form into the MAD office. 

Fill out and sign the medical release form/field trip form which can be found here. Turn this form into the MAD office. 

Group Interviews in the Academy are mandatory following the Student Application. The next Group Interview will be held on March 17, at 5:00 pm in the Academy. To reserve your space, please contact madacadapp@gmail.com.
