Required Supplemental Information

Parents of applicants who are not currently enrolled at Santa Barbara High School are required to include a transcript from their child’s current school with this application. 

Please provide three letters of recommendation. At least two recommendations should be from teachers. Please use the forms provided. Include the person’s name, position, signature, and contact information. These forms can be submitted here.

Please read and Sign the attached “Parents Agreement to Participate” and “Academy Members Learning Agreement.” Which can be found here. Turn this form into the MAD office. 

Fill out and sign the medical release form/field trip form which can be found here. Turn this form into the MAD office. 

Group Interviews in the Academy are mandatory following the Student Application. The Group Interview dates will be posted on the MAD website and will be held in the Academy. To reserve your space, please contact madacadapp@gmail.com.
